We are seeking a dedicated Registered Nurse (RN) to support residents within an assisted living community while also providing home health and hospice services in the same setting. This is a unique opportunity to combine leadership, clinical oversight, and hands-on patient care—all in one location.
What You’ll Do
- Oversee and coordinate clinical care for assisted living residents
- Conduct initial and ongoing assessments, including move-in evaluations and service plans
- Collaborate with the interdisciplinary team (IDT) to develop individualized care plans
- Provide home health and hospice nursing care to residents within the community
- Communicate effectively with physicians, families, facility staff, and other providers
- Ensure all physician orders are implemented and followed up appropriately
- Monitor resident health status and adjust care as needed
- Maintain timely, accurate documentation
- Train and support caregiving staff through orientation and competency validation
- Ensure compliance with state regulations and clinical standards
- Assist residents and families in accessing resources that improve quality of life
What We’re Looking For
- Active RN license
- Experience in home health, hospice, or assisted living preferred
- Strong clinical judgment and critical thinking skills
- Ability to collaborate and communicate effectively across care teams
- A compassionate, patient-centered approach—this is a must
