Responsibilities
- Assist patients post-hospitalization with appointment scheduling and medication management.
- Educate patients on their disease process and provide comprehensive health education.
- Identify and address concerns related to the patient's diagnosis and social determinants of health (SDOH).
- Prevent readmissions by proactively addressing patient needs and providing necessary support.
- Collaborate with the Atlanticare Primary Care Physican Group to ensure a coordinated care approach.
- Maintain accurate and up-to-date patient records and documentation.
- Communicate effectively with patients, their families, and the healthcare team to ensure a holistic approach to care.
- Stay informed about the latest advancements and best practices in transitional care.
- Provide emotional support and guidance to patients during their recovery process.
- Registered Nurse (RN) license in good standing.
- A minimum of 2 years of experience in a healthcare setting, preferably in transitional care or primary care.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients.
- Strong organizational and time management skills, with the ability to prioritize tasks.
- Empathy and a patient-centered approach to care.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in using electronic health records (EHR) systems.
- Flexibility to work remotely and in-office as needed.
- Current CPR certification is preferred.
- A passion for patient advocacy and a commitment to continuous learning.
