What are the responsibilities and job description for the Group Support Facilitator position at AtlantiCare?
POSITION SUMMARY
The Group Support Facilitator (GS) is a vital member of the team facilitating the Intensive Cardiac Rehabilitation (ICR) Program at the Life Center. The GS is responsible for implementing love and support element of ICR and facilitating the power of connection within each cohort.
Principle duties include: participates in the screening of participants and evaluates psychosocial surveys to ensure appropriate and successful group participation; facilitates and delivers group support to participants per program guidelines and protocol; provides support to participants in developing the skills necessary for integrating lifestyle changes into daily life; monitors group support adherence for all participants and provides additional explanation of guidelines on an individual basis as needed; attends weekly team meetings/case conferences; documents and communicates participants response to interdisciplinary treatment plan.
Ensures that the ICR program provides high level of participant satisfaction; attends and participates in mandatory monthly element specialist calls; complies with HIPAA privacy and patient confidentiality; and meets/maintains accreditation standards as required by the Dr. Ornish Program for Reversing Heart Disease® and provides support to alumni community as needed.
QUALIFICATIONS
EDUCATION: Master’s degree required in Behavioral Health or related field.
LICENSE/CERTIFICATION: American Heart Association Basic Life Support Certification required within 30 days of hire or transfer. Licensed mental health professional (psychologist, psychiatrist, licensed clinical social
worker (LCSW), or other mental health professional (LPC, LMFT, etc.)required.
EXPERIENCE: Two (2) years in behavioral health setting. Experience and adeptness in working with groups. Ability to facilitate a process of self-expression and active listening in a way that can create a connection, community and intimacy among group participants. Ability to model and promote verbalization of feelings, and an empathic listening presence. Experience or knowledge of medical terminology, basic anatomy and physiology, cardiovascular disease, working with the elderly or individuals with physical limitations/special needs, and/or the willingness to learn.

WORK ENVIRONMENT
Position requires considerable walking and standing.
