Position Summary
The Social Worker I will provide case management services to frail adults aged 65 and older to prevent premature hospitalization and institutionalization. The position supports the Senior Care Program, which serves high-risk older adults who may be vulnerable to abuse, neglect, self-neglect, and financial exploitation.
This role requires strong assessment, documentation, and coordination skills to ensure clients receive appropriate services and support to safely remain in the community.
Primary Duties
The Senior Care Program continues to experience increasing referrals each year. Senior Care funding is designed to prevent premature hospitalization and institutionalization. Clients served through this program are often identified as high-risk for abuse, neglect, self-neglect, and financial exploitation. This position is critical in supporting vulnerable older adults to safely remain in the community.

Primary Duties
- Provide comprehensive case management services to frail seniors aged 65 and older.
- Conduct psychosocial assessments and complete detailed written summaries.
- Perform semi-annual eligibility redeterminations for Senior Care clients.
- Identify signs of abuse, neglect, self-neglect, and financial exploitation, and make referrals to Adult Protective Services (APS) as appropriate.
- Facilitate and monitor the utilization of Senior Care funds for approved services and ensure linkage to community resources and supports.
- Bachelor’s degree in Social Work or a related human services field (MSW preferred).
- Experience providing assessment and case management services to frail elders and/or individuals with disabilities.
- Experience conducting psychosocial assessments and writing detailed case documentation.
- Knowledge of community-based resources and service systems.
- Strong written and verbal communication skills.
- Ability to maintain accurate electronic case management documentation and utilize required technology systems.
- Excellent organizational and time-management skills
- Strong analytical and assessment abilities
- Ability to manage high-risk and vulnerable populations
- Professional judgment and ethical decision-making
- Ability to work independently and collaboratively within a multidisciplinary team

- Prolonged periods sitting and working on a computer.
- Ability to lift/move up to 25 lbs.
- Standard hours: Monday–Friday, 8:30 a.m.–5:00 p.m.; evening/weekend hours may be required.
