Schedule

Flexible – We are open to both full-time and part-time candidates.

• Full-Time: 30–40 hours per week

Position Summary

The Licensed Certified Social Worker – Clinical (LCSW-C) provides clinical social work and medical case management services supporting individuals receiving HIV services through Montgomery County Department of Health and Human Services.

This role focuses on assessing client needs, developing individualized service plans, supporting treatment adherence, and coordinating care across healthcare providers and support services to ensure continuity of care and improved health outcomes.

Primary Duties

• Complete initial bio-psychosocial assessments and develop individualized service plans based on identified client needs.
• Conduct continuous monitoring and reassessment of clients at least every six months, updating service plans as necessary to ensure effectiveness.
• Provide treatment adherence counseling to support client readiness for and adherence to complex HIV treatment regimens.
• Coordinate care to ensure timely access to healthcare providers and supportive services.
• Maintain accurate documentation and collaborate with interdisciplinary healthcare teams to support comprehensive client care.
  
  

Minimum Qualifications

• Licensed Certified Social Worker – Clinical (LCSW-C)
• Minimum 3 years medical case management experience
• Minimum 3 years experience writing psychosocial assessments
• Experience completing behavioral health screenings
• Experience working in a healthcare or clinical setting
• Experience collaborating with interdisciplinary care teams
