Licensed Practical Nurse (LPN) - LTC Long Term Care / Geriatrics
Location: Kane, PA
Pay: $26 - $26.75 hourly
Shift: 6:00 PM-6:00 AM Nights 6PM - 6AM // Two days on // Two days off // Three days on // 84-hours biweekl
Overview:
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join the LTC Long Term Care / Geriatrics team of a respected healthcare organization. This role is ideal for healthcare professionals looking to work in a collaborative environment that prioritizes patient-centered care.

Key Responsibilities/Job Requirements:
- Carry out the medical plan of care under the direction of the RN Supervisor
- Oversee Nursing Assistants' care of residents to ensure compliance with good nursing practice and applicable State and Federal regulations
- Administer and record medication, treatments and monitoring as ordered
- Accurately transcribe physician's orders
- Complete monthly summaries as scheduled
- Review care plans with Nursing Assistants and update to reflect resident's current condition
- Notify RN Supervisor immediately of significant changes in resident condition
- Report and record changes in resident condition to RN Supervisor
- Communicate resident transfers to immediate family and department heads
- Complete transfer forms for all transfers
- Update care plans with clear, concise, time-limited goals
- Assist physicians with rounds, examinations and treatments as needed
- Collect laboratory specimens and arrange for delivery with accurate requisitions
- Supervise and assist with post mortem procedures when needed
- Encourage resident participation in scheduled activities
- Assist RN Supervisor with annual evaluations of nursing assistants
- Write commendations and counseling records as needed
- Complete incident reports or assist team members in writing reports
- Document weights and vital signs and follow up as needed
- Clean and stock treatment trays daily
- Prepare daily reports for the wing
- Count narcotics with another nurse at beginning and end of shift
- Maintain accurate inventory of resident belongings
- Maintain work area in clean, orderly condition
- Submit maintenance requests as needed
- Maintain a clean, safe and healthy environment
- Supervise and direct resident care by nursing assistants
- Attend care plan conferences for residents on assigned wing if requested
- Assist in orientation of new nursing staff
- Adjust staffing assignments as needed to ensure good resident care
- Conduct team conferences as needed
- Update daily census as needed
- Monitor NA documentation for completeness and regulatory compliance
- Monitor nurse aide job performance for infection control compliance
- Provide direct care for residents as needed within restrictions
- Attend in-services, department and staff meetings, committee meetings as required
- Assist CNAs on the hall as needed
Qualifications:
- Active Licensed Practical Nurse (LPN) license in good standing (state-specific)
- Minimum of 1-3 years of relevant LTC Long Term Care / Geriatrics experience preferred
- BLS certification required; ACLS preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and teamwork abilities
