Key Responsibilities
- Coordinate patient care and caregiver schedules
- Communicate with patients, families, caregivers, and healthcare professionals
- Manage schedule changes, call-offs, and staffing needs
- Follow up on patient needs, concerns, and satisfaction
- Maintain accurate documentation and schedules
- Assist with intake and service coordination
- Protect patient confidentiality and follow HIPAA requirements
- Spanish-speaking is a MUST
- Previous home healthcare/home care experience REQUIRED
- Experience with patient care coordination, scheduling, intake, or staffing
- Excellent communication and organizational skills
- Ability to multitask in a fast-paced environment
- Proficient with computers and healthcare/scheduling software
