Key Responsibilities
- Case Management & Advocacy: Oversee the development, implementation, and ongoing monitoring of Individual Support Plans (ISPs). Ensure these plans accurately reflect the individual's needs, preferences, and long-term goals.
- Regulatory Compliance: Maintain thorough, up-to-date documentation in accordance with Pennsylvania ODP regulations and agency policies. Prepare for and participate in licensing audits and internal quality reviews.
- Stakeholder Collaboration: Serve as a central point of contact for families, support coordinators, direct support professionals, and medical providers to ensure coordinated care.
- Crisis Intervention & Support: Respond promptly to critical incidents, ensuring the safety and well-being of the individuals we serve. Facilitate proactive problem-solving to mitigate ongoing risks.
- Team Leadership: Provide guidance and support to direct care staff regarding implementation of support plans and best practices for IDD care.
- Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a closely related field.
- Experience: Minimum of 2–3 years of experience in human services, specifically working within the IDD population and the Pennsylvania ODP system.
- Technical Skills: Proficiency in HCSIS (Home and Community Services Information System) and standard electronic record-keeping software.
- Soft Skills: Strong communication, organizational, and conflict-resolution skills. A warm, patient, and professional demeanor is essential.
- Requirements: Valid driver’s license and reliable transportation for regional travel. Ability to pass required background checks and clearances.
