Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Monitor patient health by taking vital signs and observing for any changes in condition or behavior
- Administer medications accurately following medical documentation protocols and HIPAA regulations to ensure patient safety
- Support clients with meal preparation, cooking, laundry, cleaning, and maintaining a clean living environment
- Document all care activities meticulously using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies
- Assist with medical procedures such as catheter care, hospice support, or working with patients who have developmental disabilities or disabilities requiring specific interventions
- Use medical terminology confidently when communicating with healthcare professionals and documenting patient records
- Employ proper safety protocols during patient care activities, including CPR certification and safe handling of equipment like Hoyer lifts
Skills
- Strong patient observation skills to detect changes in health status promptly
- Experience working in assisted living facilities, nursing homes, or home health environments
- Familiarity with EMR/EHR systems for accurate documentation of patient care activities
- Knowledge of caring for patients with dementia, Alzheimer’s disease, or memory impairments
- Ability to assist with complex medical needs such as medication administration and catheter care
- Compassionate caregiver with excellent communication skills to work effectively with clients and their families
- Understanding of HIPAA regulations to ensure confidentiality and privacy of patient information
- Proven ability to work independently while adhering to safety standards and care plans
- Experience providing home & community-based care services to diverse populations including seniors and people with disabilities
