Role Description The Community Liaison is responsible for building and maintaining relationships with local healthcare providers, community organizations, and referral sources to promote AsterCare’s home health services. This on-site role involves regular visits to hospitals, clinics, nursing homes, adult daycare centers, senior centers, and other community locations to conduct outreach, present service offerings, and support client referrals. Day-to-day tasks include coordinating community events, providing education about home care options, responding to inquiries from prospective clients and families, and ensuring a positive experience for partners and stakeholders. The Community Liaison collaborates with internal care teams to align outreach efforts with service capabilities and helps identify opportunities to expand AsterCare’s presence in the community.

- Excellent Communication and Customer Service abilities to interact effectively with clients, families, and professional stakeholders.
- Understanding of Home Care services and basic healthcare concepts to accurately represent AsterCare’s offerings.
- Relevant experience in community relations, marketing, social services, or healthcare outreach preferred.
- Ability to work on-site in southeast Michigan travel locally for meetings and events, and manage schedules independently.
- Proficient in basic office software and documentation; organized with strong follow-up and relationship management skills.
- Associate diploma or Higher degree required; additional education in healthcare, social work, communications, or a related field is beneficial.
