As a Home Health Medical Social Worker (MSW), you will provide critical support and resources to patients and their families in the comfort of their own homes. If you are a social worker with a passion for improving the quality of life for those in need, this may be the perfect opportunity for you!
Your key responsibilities include:
- Provide emotional and practical support to patients and their families in their homes
- Assess patients' needs and develop individualized care plans
- Link patients with community resources and services
- Collaborate with interdisciplinary healthcare teams to provide holistic care
- Maintain accurate and up-to-date patient records
- Stay current with advancements in home health care and social work practices
Must Have:
- Bachelor's or Master's degree in Social Work
- At least one year of Medical Social Work experience; home health experience preferred
- Experience in health care, home care or home health care settings
- Ability to work independently and manage caseload
- Strong understanding of health care systems and policies
- Proficiency in electronic medical records (EMR) systems
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines
- Ability to effectively utilize EMR and other technology to maintain HIPAA privacy, continuity of care, and team communication.
Skills & Attributes:
- Strong critical thinking skills when working independently in the field.
- Excellent interpersonal skills to build rapport with diverse patient populations.
- High level of organization and time management to navigate a daily route.
Nice to Have:
- Experience in acute care and working in a home environment.
- Experience with KanTime EMR.
