Association House of Chicago is currently hiring a Case Manager – Community Mental Health to join its team in Chicago, IL. This is a Monday – Friday, entry-level role with flexible first-shift hours, offering a meaningful opportunity to launch or grow your career in community mental health.
As a Case Manager, you will use outreach strategies to engage participants with moderate mental illness and/or co-occurring disorders. You will deliver individualized interventions grounded in each participants’ strengths, resources, preferences, and choices.

Job Duties:
- Support participants in identifying and building natural supports for treatment and recovery using culturally competent practices
- Provide crisis management services using de-escalating principles
- Complete all documentation of services in an accurate, complete, and timely manner
Job Requirements:
- HS Diploma/GED required PLUS 5 years of experience in mental health or human services OR Bachelor’s degree from an accredited college or university in psychology, social work, or related field preferred
- Valid Illinois driver’s license and insurance with an acceptable driving record, required
- Experience working with diverse ethnic populations, required
- Exposure to Medicaid billing and documentation, preferred
- Bilingual Spanish/English, preferred
