Associates Home Health is seeking an experienced Licensed Vocational Nurse (LVN) to serve as a Patient Care Coordinator. This is a hybrid clinical and administrative role that sits at the center of our care delivery operations. The LVN Patient Care Coordinator acts as the primary liaison between patients, field clinicians, physicians, referral sources, and the administrative team — ensuring every patient receives timely, well-coordinated, high-quality skilled home health services. The ideal candidate brings hands-on nursing experience, strong organizational skills, and a passion for patient advocacy.
Key Responsibilities
Care Coordination and Patient Management
- Oversee the onboarding and intake process for new patients, ensuring timely start of care following physician referral
- Schedule and coordinate skilled nursing, physical therapy, occupational therapy, and speech therapy visits
- Monitor patient caseloads and visit frequencies to ensure compliance with physician orders and plan of care
- Identify and escalate changes in patient condition to the Director of Nursing or supervising RN promptly
- Conduct or assist with telephonic patient follow-up calls and care check-ins between visits
Physician and Referral Liaison
- Communicate with physicians' offices to obtain, clarify, and track orders, 485s, and plan of care certifications
- Serve as a point of contact for referral sources including hospitals, SNFs, clinics, and case managers
- Facilitate smooth transitions of care for patients discharging from hospital or facility settings
Documentation and Compliance
- Ensure clinical documentation is complete, accurate, and submitted within agency and Medicare/Medicaid timelines
- Track OASIS submission windows, recertification periods, and discharge timelines
- Maintain patient records in the electronic medical records (EMR) system with attention to accuracy and timeliness
- Support the agency's QAPI initiatives, survey readiness, and regulatory compliance efforts
Field Support and Team Collaboration
- Collaborate daily with field nurses, therapists, aides, and the clinical supervisor to address care gaps or coverage needs
- Assist with scheduling adjustments due to staffing changes, patient cancellations, or urgent add-ons
- Participate in interdisciplinary team (IDT) meetings and care conferences as needed
- Perform field visits within LVN scope of practice when operationally needed
Qualifications
Required
- Current, unencumbered LVN license in the state of Texas
- Minimum 2 years of clinical nursing experience; home health or post-acute care strongly preferred
- Current CPR/BLS certification
- Knowledge of Medicare and Medicaid home health conditions of participation (CoPs)
- Proficiency with EMR/EHR systems, specifically Kinnser / WellSky
- Exceptional communication, multitasking, and organizational skills
- Valid Texas driver's license and reliable transportation
Preferred
- Prior experience in a care coordination, case management, or patient navigator role
- Familiarity with OASIS documentation and CMS home health billing requirements
- Experience working with VA patients or veteran-focused care programs
- Bilingual (English/Spanish) a plus
Compensation and Benefits
- Salary Range: $50,000 - $60,000 annually, compensation based on experience
- Monday through Friday schedule; 8:00 AM - 5:00 PM

Work Environment
This is primarily an office-based position at our San Antonio location, with occasional field visits within the service area. Standard business hours apply; some flexibility may be required to support patient care needs. This role reports to the Director of Nursing or designee.
