Position Overview
We are seeking a skilled and compassionate Physical Therapist (PT) to join our home health team. In this role, you will evaluate patients, develop individualized care plans, and provide therapeutic interventions in the comfort of patients’ homes. You will play a critical role in improving patient mobility, function, and independence while delivering personalized, high-quality care

You will play a critical role in improving patient mobility, function, and independence while delivering personalized, high-quality care
Key Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Provide skilled physical therapy services in accordance with physician orders
- Regularly reassess patient progress and modify treatment plans as needed
- Supervise and collaborate with Physical Therapy Assistants (PTAs)
- Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
- Complete accurate and timely documentation in compliance with regulatory standards
- Coordinate care with interdisciplinary team members, including nurses, physicians, and other therapists
- Travel to patient homes within assigned service area
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Current Physical Therapist license in FL
- Minimum of [1 year preferred] experience (home health experience preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong clinical decision-making and problem-solving skills
- Excellent communication and organizational abilities
- Ability to work independently and manage a flexible schedule
