We are seeking a compassionate and dedicated Part-Time Registered Nurse (RN) to join our growing home health care team. As an RN in our agency, you will provide high-quality, personalized care to patients in the comfort of their homes. You will work closely with physicians, caregivers, and other healthcare professionals to create and execute individualized care plans, manage patient conditions, and improve overall quality of life.
Key Responsibilities:
- Provide skilled nursing care to home health patients, including assessments, treatments, and medication administration.
- Collaborate with healthcare providers to develop and implement care plans based on individual patient needs.
- Educate patients and their families on disease management, medications, and home care techniques.
- Monitor and document patient progress, health changes, and outcomes using electronic medical records (EMR).
- Ensure adherence to established policies, procedures, and regulatory requirements.
- Provide guidance and support to home health aides, as needed.
- Maintain open communication with patients, families, and care teams to ensure the highest standard of care.
- Participate in case conferences and quality assurance activities.
Qualifications:
- Active and unrestricted Registered Nurse (RN) license in Florida.
- Minimum of 1 year of clinical nursing experience, preferably in home health or a related field.
- Strong understanding of home health care regulations and best practices.
- Proficient in electronic medical record (EMR) documentation.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and manage multiple patient cases effectively.
- Valid driver’s license and reliable transportation.
