- Conduct initial and ongoing assessments of residents' physical and emotional needs
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with other members of the healthcare team to ensure effective communication and coordination of care
- Provide education and support to residents and their families on various health-related topics
- Participate in quality improvement initiatives to continually improve the quality of care we provide
