We are seeking a dedicated and compassionate Social Services Director to join our team. The ideal candidate will play a vital role in ensuring the psychosocial well-being of our residents by addressing their emotional, social, and psychological needs. This position requires a strong commitment to advocacy, effective communication, and fostering positive relationships with residents, families, and staff.
Social Services Director Responsibilities:
Assess and address the psychosocial needs of residents upon admission and throughout their stay.
Develop and implement individualized care plans in collaboration with interdisciplinary teams.
Serve as a liaison between residents, families, and staff to facilitate communication and resolve concerns.
Provide emotional support, counseling, and crisis intervention to residents and families.
Coordinate discharge planning and ensure a smooth transition for residents returning to the community or moving to another care setting.
Advocate for residents' rights and ensure compliance with federal, state, and local regulations.
Maintain accurate documentation of all assessments, care plans, and interventions.
Organize and facilitate support groups and educational programs for residents and families.
Stay updated on community resources and connect residents/families to appropriate services.
Social Services Director Qualifications:
Current SSD Certificate or willingness to obtain.
Previous experience in a long-term care or healthcare setting is highly desirable.
Strong knowledge of federal and state regulations related to long-term care facilities.
Excellent communication, organizational, and interpersonal skills.
Ability to work collaboratively as part of an interdisciplinary team.
