Position Details:

- Setting: Home Health | Office Position

WHAT YOU’LL BE DOING
In this role, you are the heartbeat of home health operations—leading clinical excellence while keeping your team aligned and supported.
- Lead with purpose: Oversee and guide individualized care plans to ensure optimal patient outcomes
- Drive quality: Review OASIS, 485s, and clinical documentation for accuracy, timeliness, and compliance
- Coordinate seamlessly: Manage referrals, case conferences, and interdisciplinary collaboration
- Ensure patient safety: Monitor clinical indicators, wound care progress, and changes in condition
- Support your team: Step in for visits or on-call coverage when needed
- Own the process: Oversee orders, authorizations, and administrative workflows to keep operations running smoothly
WHAT WE’RE LOOKING FOR:
- Active RN license in Oregon
- 2 years RN experience, including 1 year in home health
- MUST possess STRONG OASIS & HomeCare HomeBase (HCHB) proficiency
- Proven leadership, organization, and communication skills
- Prior clinical management or supervisory experience
- Ability to multi-task in a fast-paced environment without compromising care quality
- Valid driver’s license, auto insurance, and willingness to travel in varying conditions
