Job Responsibilities:
- Coordinate patient care from injury through recovery.
- Conduct assessments and develop personalized care plans.
- Collaborate with healthcare providers, insurance companies, and patients.
- Travel to meet patients while managing administrative tasks from the office or home.

Key Responsibilities for Ascendant Medical Case Managers:
- Conduct ongoing medical assessments during patient interactions.
- Establish and coordinate treatment plans with measurable short-term and long-term goals.
- Implement treatment plans safely and effectively, modifying them as necessary throughout recovery.
- Communicate efficiently with members of the patient care team.
- Monitor changes to treatment plans based on provider recommendations.
- Collaborate continuously with patients, families, physicians, and caregivers to ensure high-quality care.
- Support patients in informed decision-making regarding their clinical status and treatment options.
- Advocate for patients’ needs.
- Attend all necessary medical appointments related to the accident (e.g., doctor’s appointments, therapy evaluations).
- Maintain comprehensive medical records for all patients.
- Keep appointment and provider trackers updated in the online patient profile.
- Notify the Integrated Case Manager (ICM) of any changes in appointments promptly to coordinate care effectively.
- Ensure timely uploading of monthly reports and documentation into Myjunna.
- Complete all required patient-related documentation, including Physician Office Visit Forms and Medical Release Forms, on time.
- Use therapeutic communication to address psychosocial challenges that accompany injury and illness.
- Adhere to professional standards outlined by Ascendant’s protocols and regulations.
- Develop discharge plans upon completion of case management services.
- Communicate with the Marketing Team’s Senior Medical Case Manager regarding any referrals.
- Introduce yourself to adjusters and attorneys, keeping them informed of any changes.
