The Social Worker is responsible for, but not limited to:
- Initiates action and obtains resolution. Responds promptly, professionally and patiently. Work Collaboratively.
- Nurture by supporting all customers. Acts with courtesy, care, concern and compassion. Educates others by sharing knowledge and expertise.
- Greets customers in a friendly and welcoming manner. Creates a positive first impression. Smiles, projects a positive attitude.
- Through the continuous improvement of standards of practice and by providing a safe and secure environment the customer will be assured a sense of safety and well-being, free from risk and harm.
- Contributes to the effectiveness of the department/Agency by remaining within the acceptable standard of the Agency's Attendance/Tardiness Control Policy.
- Provides direct social work services in the home which meet Agency and professional standards. Meets Agency standards of competency.
- Accurately documents care provided and maintains patient records according to Agency policies.
- Assists with Agency and community programs/committees.
Master's Degree in Social Work
Professional Experience:
- Current Social Work License in the State of New Jersey, LCSW preferred
- 1 year of post graduate experience in a health setting
