The Licensed Practical Nurse (LPN) is responsible for, but not limited to:
- Observe and document the physical, emotional, and psychosocial needs of patients and their families.
- Collaborate with the Hospice RN regarding patient status, symptom management, and changes in condition.
- Provide skilled nursing care as directed by the attending physician and supervised by the Hospice RN.
- Monitor patient responses to treatment and document all care provided.
- Maintain continuity of care through effective communication with patients, families, physicians, and the interdisciplinary team.
- Participate in Interdisciplinary Team (IDT) meetings and other required clinical meetings.
- Maintain adequate home care supplies to support quality patient care.
- Assist with collecting clinical documentation and required paperwork.
- Support clinical billing activities as needed.
- Assist with the orientation and training of new Hospice Aides when assigned.

Education
- Associate Degree in Nursing or a related field preferred.
- Current, unrestricted LPN license in the state of practice.
- Minimum of one (1) year of clinical nursing experience.
- Hospice, Home Health, or Long-Term Care experience preferred.
- Strong verbal and written communication skills.
- Ability to work effectively as part of an interdisciplinary team.
