Job Description
The Lead Housing Case Manager is a leadership position within Ascencia’s Housing Case Management team. In addition to performing all duties of a Housing Case Manager, the Lead ensures that the team delivers high-quality, client-centered services to clients in permanent supportive housing (PSH), primarily serving Glendale COC clients who meet or have met the HUD definition of chronically homeless. The Lead Housing Case Manager also supports the CES Manager by monitoring case manager workloads, reviewing client files, conducting audits, and providing training and mentorship to new and existing staff.
Duties and Responsibilities
Administrative / Leadership

Perform all duties of a Housing Case Manager.

Monitor PSH programs:

Periodically review client files to ensure proper completion of annual assessments and accurate rent payments.

Assist CES Manager in annual PSH monitoring and reporting.

Plan, coordinate, organize, review, and audit case manager workflow.

Adjust tools and documentation for the program as needed.

Monitor home visits to ensure they are completed in a timely manner.

Train, mentor, and support new staff in housing case management protocols.
Access Center / CES Reallocation & Emergency Housing Intakes

Train staff on completing Access Center, CES Reallocation, and Emergency Housing Intakes.

Provide guidance and mentorship to newly hired staff on intake processes.
Client Satisfaction & Engagement

Verify completion of annual client satisfaction surveys for all PSH clients.

Ensure case management practices are client-centered and aligned with Ascencia’s service standards.
Qualifications and Requirements

Must currently be a CES Team Case Manager.

Bachelor’s degree in human services or comparable combination of education and work experience required.

Experience in a social service setting with knowledge of case management systems and planning techniques.

Knowledge of homeless and housing-related resources, housing rights, and the Continuum of Care preferred.

Experience working with chronically homeless individuals, clients with chronic health issues, and substance use disorders preferred.

Must attend all mandatory meetings and trainings required for the position.

Proficient in Microsoft Word and Excel; ability to learn and use client database for recordkeeping.

Must have own transportation and meet insurance requirements to drive agency vehicles.

Valid driver’s license and active auto insurance required.
Physical Requirements

Ability to travel for job-related duties, including home visits and client meetings.

Use of computer and phone for documentation and communication.
