Job Description
This temporary role is expected to continue through September 30, 2026, at which time the assignment will end. The General Case Manager role works with homeless individuals and families to develop personalized case management plans that address barriers to permanent housing. The General Case Manager provides support in areas such as benefits and healthcare advocacy, budgeting, housing and employment planning, education support, and connections to community resources as needed. This position works closely with Housing Case Managers and program staff to ensure comprehensive, client-centered services that promote long-term housing stability.

Salary: $22.00 per hour
Duties and Responsibilities
Case Management

Assess the service needs of prospective clients and develop individualized service plans.

Provide ongoing support to clients in achieving their housing, employment, and personal goals.

Conduct crisis intervention and provide guidance in urgent situations.

Adhere to professional ethics and maintain strict boundaries in client interactions.

Prepare case management reports, including outcomes and program successes.
Housing Counseling & Placement Support

Work with clients to develop individualized housing plans.

Assist clients with housing applications and supportive/subsidized housing paperwork.

Survey the rental market for affordable housing and advocate with prospective landlords.

Support housing counseling and coordinate moves to permanent housing alongside Housing Case Managers.
Documentation & Administrative Responsibilities

Maintain accurate documentation of service objectives, client outcomes, and program activities in accordance with Federal, State, County, and agency guidelines.

Maintain client data systems, including HMIS entries and case notes.

Attend program-related staff, community, coalition, and committee meetings as assigned.

Research and maintain up-to-date records of providers and resources.
Case Conferencing

Coordinate work with Ascencia staff and other partners to ensure effective service delivery.
Other Duties

Perform other duties as assigned to support program and organizational goals.
Qualifications and Requirements

Bachelor’s degree in human services or a related field, or equivalent combination of education and experience.

Experience in social service or homeless services settings with knowledge of case management systems and planning techniques.

Knowledge of homeless and housing-related resources, tenant rights, and the Continuum of Care preferred.

Experience working with homeless individuals and families preferred.

Proficient in Microsoft Word and Excel; ability to learn and use client database systems for recordkeeping.

Must have reliable transportation, valid driver’s license, and meet agency insurance requirements.
Physical Requirements

Travel to field locations to carry out job responsibilities.

Prolonged sitting, standing, bending, stooping, stretching, and occasional lifting (up to 25 pounds).

Frequent use of computer, keyboard, and phone for documentation and communication.
