Position Name: Nurse Navigator (RN) | Patient Concierge
Location: Santa Monica, CA
Employment Type: Direct-Hire & Permanent
Setting: Private Practice / Startup Concierge Healthcare
Pay: Competitive; based on experience
Shift: Full-Time | Mon-Fri | 8am-5pm (flexibility required; potential after-hours support)
Requirements:
- Registered Nurse (RN) or Nurse Practitioner (NP) (preferred).
- Active RN/NP license.
- Oncology experience (strongly preferred).
- Strong communication and patient coordination skills.
- Ability to work in a fast-paced, evolving environment.
- Experience in a concierge or patient navigation setting.
- Comfortable supporting high-touch, concierge-level patient care.
- Clinical trials exposure.
- Assist on the clinical side when needed, such as helping with ports and IVs.
Job Summary:
- Guide patients through their care journey, especially when transitioning off treatment or clinical trials.
- Coordinate referrals, follow-ups, and continuity of care across providers.
- Support concierge patients and families, ensuring a high-touch, seamless experience.
- Assist across both clinical and research functions as needed.
- May step in on the clinical side when needed (e.g., ports, IVs).
- Collaborate closely with the care team to ensure alignment on patient needs and next steps.
- Adapt to a fast-paced environment where patient needs and care plans may change quickly.
