The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA's 8 Dimensions of Wellness-emotional, social, physical, financial, and more.
You'll work as part of a multi-disciplinary team, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.
- Build strong, therapeutic relationships with clients-including those who may be harder to engage
- Help clients set and achieve personalized wellness goals
- Provide skill-building and psychoeducational support (individual & group settings)
- Coordinate care plans and connect clients to essential community resources
- Support clients and families in making informed decisions
- Assist with access to medical, behavioral health, and social services
- Provide crisis intervention when needed
- Maintain accurate documentation and meet service delivery standards
- Strong interpersonal and communication skills
- Passion for mental health, community support, and advocacy
- Ability to work collaboratively in a team environment
- Flexibility and willingness to meet clients where they are (including community-based work)
- Experience with behavioral health, case management, or human services preferred
This is a dynamic, community-based role requiring travel to client homes, service sites, and community locations. Work may be conducted in-person or via telehealth, depending on client needs.
Schedule
- Full-time, Monday-Friday (8:00 AM - 5:00 PM)
- Occasional evenings or extended hours as needed
- Ability to sit, stand, walk, and use standard office equipment
- Occasionally lift up to 25 lbs

Requirements
- The Adult Behavioral Health Home Coordinator/Community Integration Counselor requires an associate degree or equivalent credits in a Human Services field.
- Bachelor's degree preferred. Must be certified at the full MHRT-C or provisional level. Use of personal vehicle and active driver's license is required.
