As a Population Health Specialist, you’ll work directly with clinical and administrative teams to support population health performance, ensure compliance with quality initiatives, and help drive improved patient outcomes. Reporting to the Executive Director of Quality, you will serve as a liaison between internal departments and external partners such as insurers and regulatory agencies. Your work will support key programs such as PCMH, DSRIP, and Medicare Wellness initiatives.
Your Schedule
- Full-Time, 40 hours per week
- Day Shift, Monday through Friday
- 6 Paid Holidays
- Collaborate with the CIS and MIS teams and clinical users to ensure accurate data capture for reporting.
- Support and optimize EMR workflows through analysis and stakeholder feedback.
- Conduct audits and validate documentation to ensure compliance with standardized protocols.
- Extract and analyze data from EMRs and databases like Arcadia to identify care gaps and improve population health performance.
- Monitor and report on quality scores, chronic disease management metrics, and compliance with various incentive programs.
- Develop workflows and reporting tools to support pre-visit planning, referral tracking, and gap closure strategies.
- Provide training and educational support related to quality initiatives and reporting requirements.
- Attend relevant trainings, webinars, and quality-focused meetings to remain up to date on best practices.

Requirements
- High school diploma required; medical terminology preferred.
- Minimum of three (3) years’ experience in a healthcare setting.
- Advanced proficiency in Microsoft Excel and other Microsoft applications.
- Familiarity with electronic health record systems and reporting tools.
- Ability to travel locally to practice sites as needed.
- Excellent interpersonal and communication skills.
- Strong analytical and problem-solving abilities.
