Position Summary
The Assistant Chief Population Health Officer (ACPHO) is a key physician leadership role that will drive the Guthrie Clinic’s transition to value-based care and improve the health outcomes of the populations we serve. This system-level position is focused on elevating performance on ambulatory quality metrics, leading data-informed care transformation, and integrating outreach and chronic disease management programs across the continuum of care.

- Board-certified physician with active NY and PA licensure (or eligibility).
- Minimum 5 years of leadership in an integrated health system.
- Demonstrated success in quality improvement, clinical data reporting, and care transformation.
- Background in health plans or as a medical director for value-based or quality departments is highly desirable.
