Physical Therapy Assistant

Schedule

Monday to Friday, 8 AM to 4 PM

The Physical Therapy Assistant will work under the direct supervision of a physical therapist to execute individualized treatment plans effectively and in accordance with federal and state regulations, organizational policies, and APTA guidelines. This role involves collaboration with physical therapists to develop comprehensive plans of care that promote optimal functional independence and safety.

• Implement individualized treatment plans as per clinical documentation and compliance standards.


• Collaborate with physical therapists in the development of treatment strategies.


• Conduct skilled physical therapy treatments, assessing patient status and ensuring quality care.


• Coordinate with clinical team members to enhance patient care and communication.


• Monitor and report changes in patient conditions to the case manager promptly.


• Engage in continuous communication of clinical and operational activities with physical therapists.


• Complete necessary documentation to reflect care provided and the outcomes achieved.


• Maintain and enhance personal knowledge in the field of physical therapy.


• Stay up-to-date with HHCAHPS standards integrated into visit protocols.
