What You'll Do:
- Conduct comprehensive assessments of patients’ needs in their home environment
- Develop and implement individualized treatment plans to enhance patients' ability to perform daily functions
- Provide skilled physical therapy services, adjusting interventions as necessary to achieve patient goals
- Educate patients and caregivers on strategies to improve functional mobility and independence
- Ensure timely and accurate documentation in compliance with OASIS and regulatory standards
- Collaborate with a multidisciplinary team to deliver optimal care
- Maintain positive, professional relationships with patients, families, and colleagues
Scheduling:
- Full-Time: Consistent caseload with flexible daily scheduling
Compensation:
- Pay-per-unit model
- Full-Time Rate: Starting at $70 per unit (OASIS SOC: Starting at $210, Eval: Starting at $105, Revisit: Starting at $70)
- Competitive rates based on visit type and experience
- Mileage reimbursement provided
Requirements
What You’ll Need:
- Active Physical Therapist license in Arizona
- 1-2 years of Home Health experience required
- Fluency in OASIS required (KanTime knowledge a plus)
- Arizona Level One Fingerprint Clearance Card required
- Valid Driver's License and car insurance card
- Current CPR and First Aid certification required
- Current TB test
- Two professional references required at time of interview
