Duties
- Provide attentive patient observation to monitor health status and respond promptly to changes or emergencies
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Administer medications.
- Perform medical procedures such as catheter care, vital signs measurement, and CPR when necessary
- Support clients with dementia or Alzheimer’s by implementing specialized memory care techniques and ensuring a safe environment
- Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate medical documentation and record keeping
- Utilize assistive devices like Hoyer lifts safely during patient transfers and provide hospice or palliative care as needed
- Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks including cleaning, laundry, and meal preparation
- Maintain detailed medical records, update care plans, and ensure compliance with healthcare privacy standards such as HIPAA
- Work collaboratively with healthcare professionals to coordinate home & community care services that meet individual needs
Experience
- Prior experience in assisted living facilities, nursing homes, or home health environments demonstrating strong patient care skills
- Proven ability to work with individuals with developmental disabilities or those requiring specialized dementia or Alzheimer’s care
- Familiarity with medical terminology, medication administration protocols, and vital signs monitoring.
- Long-term care experience preferred, especially in home & community settings involving caregiving for seniors or individuals with disabilities
- Knowledge of hospice care practices and working with patients facing end-of-life situations is advantageous
- Certification in First aid andCPR, finger print card and two step TB test is required;
- Ability to perform physical tasks including lifting patients using Hoyer lifts and assisting with mobility needs
