Job Title: Hospice Liaison

Location: Fort Smith

Position Summary:

We are seeking a compassionate and motivated Hospice Liaison to develop and maintain strong relationships with healthcare providers, community organizations, and referral sources throughout the service area. This position is responsible for educating professionals and the community about hospice services, identifying opportunities for growth, and increasing appropriate referrals through strategic outreach and relationship building. The ideal candidate is passionate about serving others, possesses excellent communication skills, and is committed to representing AAAWA's mission with professionalism and integrity.

Key Responsibilities:

  • Relationship Development: Build and maintain strong relationships with area hospitals, physicians, nursing homes, assisted living facilities, home health agencies, discharge planners, social workers, case managers, and other referral sources that result in steady referral streams for AAAWA.
  • Community Outreach: Establish and maintain trust with local organizations, including churches, senior centers, veteran groups, civic organizations, and other community leaders to increase awareness of hospice care and AAAWA's services.
  • Education & Awareness: Educate healthcare professionals, referral partners, patients, families, and the public on hospice philosophy, eligibility criteria, benefits, and services through presentations, meetings, and one-on-one interactions.
  • Community Events: Partner with other members of Marketing team to plan, host, and participate in workshops, seminars, health fairs, community events, and educational programs to increase community engagement and promote hospice awareness.
  • Referral Development: Identify hospice-appropriate patients early in the care continuum and work proactively to improve referral timing and increase referral conversions through relationship management and education.
  • Admissions Collaboration: Work closely with the admissions team to ensure timely follow-up on referrals and facilitate a seamless transition for patients and families entering hospice care.
  • Marketing Strategy: Develop and implement marketing plans and outreach initiatives that align with organizational goals, increase market presence, and support sustainable program growth.
  • Performance Monitoring: Analyze referral trends, territory performance, and outreach efforts to identify opportunities for growth and recommend strategies for continuous improvement.
  • Reporting & Documentation: Prepare regular reports on marketing activities, referral outcomes, and territory performance. Maintain accurate records of referral sources, community contacts, and outreach efforts.
  • Professional Representation: Represent AAAWA's mission, values, and hospice services with professionalism, compassion, and integrity in all interactions with referral partners, patients, families, and the community.
  • Other Duties: Perform additional duties as assigned to support the Hospice program and organizational objectives.

Requirements

  1. Bachelor’s degree in Marketing preferred or equivalent combination of education and experience.
  2. Minimum two (2) years of sales/marketing experience in healthcare.
  3. Valid driver’s license.
  4. Excellent verbal and written communication.
  5. Ability to proficiently use and perform key functions of MS Office programs.
  6. Ability to handle multiple and changing priorities in a fast-paced work environment.
  7. Ability to work closely and effectively with all employees and management staff.
  8. Ability to travel out of the area and overnight.

Salary : $62,000 - $80,000
