Responsibilities:
- Provide direct support and assistance to individuals with physical, intellectual, or developmental disabilities
- Implement individualized service plans in collaboration with clients and care teams
- Advocate for clients' rights, independence, and access to community resources
- Assist with daily living activities, personal care, and mobility support
- Maintain accurate documentation, case notes, and compliance records
- Coordinate with healthcare providers, families, and community organizations
- Monitor client progress and adjust support strategies accordingly
Requirements:
- Experience in disability services, social services, or a related human services field preferred
- Knowledge of person-centered care principles preferred
- Strong communication, empathy, and interpersonal skills required
- Ability to pass a background check and meet State licensing or certification requirements required
- Valid driver's license, clear driving record, and reliable transportation required
- Ability to work varied days and hours required
- CPR/First Aid certification required (will provide if needed)
- MAT: Wyoming Department of Health Medication Assistance certification required (will provide if needed)
- High school diploma or equivalent preferred, but not required
