Certified Coding Specialist- Medical Records
Qualifications
Required Certified Coding Specialist (CCS)with American Health Information Management Association (AHIMA), Certified Outpatient Coder (COC) or Certified Inpatient Coder (CIC) with American Academy of Professional Coders (AAPC). Preferred Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA).
Must have one year of experience in hospital based work relating to the Health Information Management Department, knowledge of the ICD-10-CM coding system and CPT-4 coding system and medical terminology. Typing skill is necessary. Knowledge of computer technology.
Responsibilities
Accurately codes the hospital inpatient and outpatient record by abstracting information on the face sheet.
Always scans the hospital inpatient and outpatient record for diagnostic and procedural information to optimize all Medicare, Medicaid and commercial payers.
Always uses forms appropriate to the task; demonstrates a high degree of accuracy and attendance to detail at all times.
Always searches through progress notes, history and physical, operative notes, pathology reports, nurses notes, and orders to determine final diagnoses.
Always examines all forms in the hospital inpatient and outpatient record to assure all forms contained in the record pertains to one patient (i.e. verify patient name and patient number).
Reviews hospital record for completion of Performance Measure/Indicators forms when appropriate.
Demonstrates the ability to communicate effectively and express ideas clearly.
Promptly reports physician complaints to a Coding Manager.
Demonstrates the ability to read, write, and comprehend the contents of a medical record sufficiently to code hospital inpatient and outpatient records.
Demonstrates a thorough working knowledge of ICD-10-CM, ICD-10-PCS and CPT-4 medical coding systems and APC/DRG's.
Demonstrates a thorough working knowledge of medical terminology; asks questions to clarify misunderstandings.
Demonstrates a complete understanding of all procedures concerning the sequencing of diagnoses, operations or procedures as outlined in the department procedure manual.
Demonstrates knowledge of human anatomy and physiology to interpret general medical classifications for coding inpatient and outpatient data.
