Role Description
This is a full-time on-site role for an Intake Coordinator based in Fort Lauderdale, FL. The Intake Coordinator will be responsible for facilitating the patient intake process, including reviewing referrals, obtaining necessary medical documentation, and coordinating patient admissions. They will serve as a liaison between patients, families, healthcare providers, and insurance contacts to ensure a smooth transition into hospice and palliative care services. Additional responsibilities include maintaining accurate records, processing insurance verifications, and providing outstanding customer service to stakeholders.
Qualifications
- Strong Communication and Customer Service skills to effectively interact with patients, families, and healthcare professionals
- Bilingual required
- Must be able to work weekends (both days on a rotation)
- Must be able to work Holidays
- Experience in Case Management and understanding the patient care coordination process
- Knowledge of Medical Terminology to navigate and interpret medical documentation
- Familiarity with Insurance processes, including various plans and verification procedures
- Highly organized and detail-oriented with the ability to manage multiple tasks simultaneously
- Proficiency in relevant software and database management systems
- Previous experience in, hospice, or palliative care is REQUIRED
- Pay:$21/hr
