Job Description:
We are looking for a PRN Physical Therapists to join our dedicated staff. We seek a PT who is a self-motivated healthcare professional with a passion for thinking out of the box, developing patient center clinical programming, and providing outstanding care to meet the diverse needs of our residents.
Job Summary
The ideal candidate will be responsible for evaluating, planning, and implementing individualized treatment programs for patients to improve their physical mobility and overall quality of life. This role requires a thorough understanding of anatomy and physiology, as well as the ability to work with diverse patient populations, including geriatrics and those requiring home health services. Duties
- Conduct comprehensive assessments of patients' physical abilities and limitations.
- Develop and implement tailored care plans that address individual patient needs.
- Utilize various therapeutic techniques, including trigger point therapy and orthopedic rehabilitation.
- Provide education to patients and their families about treatment plans, exercises, and recovery processes.
- Collaborate with other healthcare professionals to ensure coordinated care and effective discharge planning.
- Document patient progress accurately in compliance with regulatory standards.
- Maintain a safe and supportive environment for patients during therapy sessions.
Skills
- Strong knowledge of anatomy and physiology relevant to physical therapy practices.
- Experience in home health settings and inpatient rehabilitation is preferred.
- Proficiency in developing care plans that cater to diverse patient needs, particularly in geriatrics.
- Familiarity with trigger point therapy techniques for pain management.
- Ability to perform discharge planning effectively to facilitate smooth transitions for patients' post-treatment.
