Job Description
Oversees the Resident Care Team Members in ensuring quality resident care is provided in a manner which upholds the organization’s philosophy and wellbeing of the residents.
Duties & Responsibilities
Resident Services
- Oversee the Resident Care Team Members on duty in providing quality care and meeting the needs of residents in accordance with Ridge Care philosophy and policies
- Develop and adhere to a plan of care (DMA-3050r) for each non-PCS resident with input from the resident, family members, Resident Care Consultant, physician, and other appropriate team members
- Establish and maintain good relationships and open communication with residents, their families, physicians, and vendors
- Schedule and complete two 8 hour Med Pass shifts weekly with 3 med pass observations every other month
- Complete any new move in prescreens. Consult with Resident Care Consultant on complex care concerns. If the ALWD is not available, the Executive Director will delegate to another community team member.
- Ensure that all systems for providing and tracking resident care are completed in accordance with the Ridge Care policies and procedures
- Coordinate dining experience needs with the Dining Service Director
- Responsible for E-MAR management; assure that MARs, TARs, and ADL Logs are accurate before printing monthly and are properly filed in a timely manner
- Review and scan monthly ADL logs to PCS designee
- Education Training
- Complete pharmacy consults and collaborate with pharmacy staff and Resident Care Consultant as needed
- Submit weekly reports to Resident Care Consultant
- Perform monthly med cart audits in conjunction with Resident Care Consultant
- If Resident Care Consultant is unavailable, complete new move in and returning resident paperwork
- Work in conjunction with the Resident Care Consultant to identify and implement Shared Risk Agreements and train team members accordingly
- Responsible for incident report tracking and fall/elopement intervention implementation
- Responsible for maintaining New Order notebook, Coumadin Management notebook, and diabetic management
- Responsible for maintaining Resident Nurse Tracking Tool
- Responsible for monthly weight tracking tool and assuring that all monthly vitals are completed and care plans updated per State Regulations
- Responsible for follow up recommendations from quarterly LHPS findings
- Complete Care Level Reviews when updating Care Plan (DMA-3050r) and review with the ED
- Maintain deviation reports and ensure that EMAR documentation matches
General Management
- Ensure all required documentation and tracking systems related to resident care services are completed as required and all deadlines for completion have been met
- Review any and all communication tools used in providing resident care services daily
- Orders and Maintains Resident Care Supplies and works with the Executive Director to ensure the resident services department operates within the communities budget
- Assist with the marketing of the community including developing relationships with local referral sources
- Assume on call responsibilities on a rotational basis and that community is staffed appropriately and in compliance with state regulations
Hiring, Supervising & Training
- Work with and assist the TMSD in hiring, orienting, and training resident care employees
- Supervise all Resident Care Team Members within the community while motivating through recognition programs, training, and team building
- Conduct regular performance appraisals with team members
- Lead by example encourage teamwork, promote the residence philosophy and provide ongoing coaching to employees
- Provide an “open door” to employees, addressing any concerns or grievances they may have
- Respond to and ensure proper handling of on the job injuries in accordance with residence policies and procedures
- Assist in training of all resident care employees in accordance to residence policies, training programs, and state and federal regulations
Qualifications
Mandatory requirements
- Professional, friendly, and caring disposition with desire to work with the elderly. Experience working with memory impaired residents preferred
- Minimum one year experience in healthcare field
- Preferred one year supervisory or management experience
- Computer proficient with in-depth experience in Excel, Word & Outlook.
- Maintain a clean driving record, valid driver’s license, reliable transportation and auto insurance
- Keep all vaccines, if required, current and up-to-date
- CPR certification
- Attend required training
- Required C.N.A. and Medication Technician certification with no substantiated findings on the HCPR
- Good time management and organizational skills. Ability to prioritize and multi-task
- Effective communicator with the ability to appropriately respond to needs and concerns as needed
- Must be able to work flexible hours including evenings, weekends, special events and holidays
Knowledge requirements
- Any required resident care training
- Complete any required management training
- Maintain knowledge of federal and state regulations
- Any required PCS training
Physical requirements
To fulfill job requirements, employees must be able to perform key physical tasks for extended periods of time. When supporting, lifting, or transferring residents, proper body mechanics and safety measures, as prescribed by the community, must be used. In a typical eight-hour day, employee will:
- Stand/walk up to 8 hours a day
- Sit up to 2 hours a day
- Frequently support 70 pounds, occasionally lift/carry up to 15 pounds
- Frequently kneel, bend and reach
- Secure proper assistance for transferring and lifting of residents, as needed.
