Your Role: Making a Meaningful Difference

As a Family Service Coordinator (FSC) with Aprendamos Early Intervention, you will serve as a key partner and advocate for families navigating Early Intervention services. You will help families understand available resources, coordinate multidisciplinary services, and ensure each child's individualized service plan remains responsive to the family's priorities and needs.

This role is ideal for someone who is organized, compassionate, relationship-driven, and passionate about supporting young children and their families.

Key Responsibilities:

• Manage an assigned caseload and ensure children and families receive appropriate Early Intervention services.
• Coordinate services, meetings, referrals, documentation, and follow-up related to each family's individualized service plan.
• Build meaningful relationships with families through regular face-to-face visits and ongoing communication.
• Collaborate with therapists, providers, families, and community partners to support a coordinated, family-centered approach.
• Maintain accurate and timely documentation in accordance with program and regulatory requirements.
• Gather and evaluate relevant information to help identify changing family priorities, child needs, and appropriate services.
• Help families understand their rights, available resources, and options within the Early Intervention system.
• Use sound judgment and problem-solving skills to objectively evaluate situations and determine appropriate next steps.
• Communicate effectively with families, team members, and community partners while maintaining professionalism and confidentiality.

What We Are Looking For

• Education: Bachelor’s degree or higher in Social Work, Early Childhood Education/Development, Family Studies, or a related field.
• Family-Centered Mindset: A genuine desire to advocate for children and families and help them successfully navigate services.
• Organization: Strong attention to detail with the ability to manage documentation, deadlines, meetings, and multiple family needs.
• Communication: Excellent interpersonal and customer-service skills with the ability to communicate information clearly to a variety of audiences.
• Collaboration: Dependable team player who works effectively with families, providers, and multidisciplinary professionals.
• Adaptability: Comfortable working in a changing environment and adjusting priorities as family and program needs evolve.
