Your Role: Making a Meaningful Difference

As an Intake Coordinator / Family Services Coordinator (Intake/FSC) with Aprendamos Early Intervention, you will guide children and families through the beginning stages of New Mexico's Family Infant Toddler (FIT) Program.

You will serve as an important connection between families and the Early Intervention team—managing referrals, explaining services and family rights, coordinating evaluations and assessments, supporting the eligibility process, and helping families understand the services available to support their child's development.

This position is ideal for someone who is highly organized, compassionate, responsive, and comfortable managing multiple timelines while building strong relationships with families and professionals.

Key Responsibilities

• Manage incoming Early Intervention referrals and guide families through the intake and eligibility process.
• Contact families promptly following receipt of a referral to introduce Early Intervention services and schedule the initial visit.
• Explain the NM FIT Program, available services, family rights, costs associated with services, and what families can expect throughout the intake process.
• Conduct initial face-to-face visits with families and gather required intake information.
• Coordinate and schedule Comprehensive Multidisciplinary Evaluations and other assessments needed to determine eligibility.
• Maintain accurate documentation regarding evaluation appointments, family communication, and intake activities.
• Request relevant medical records from the child's primary care or other healthcare providers as needed.
• Maintain regular communication with families throughout the intake and evaluation process, answering questions and addressing concerns.
• Review completed evaluation information with families and explain recommendations and potential next steps.
• Support eligible children and families with enrollment into Early Intervention services based on identified needs.
• Coordinate the intake, evaluation, assessment, and eligibility determination process in accordance with NM FIT Program requirements.
• Enter and maintain accurate child and family information within required Early Intervention database systems.
• Complete all required intake documentation, service records, and billing documentation accurately and within established deadlines.
• Submit complete and accurate documentation to the billing department in accordance with FIT Program requirements.
• Collaborate with therapists, developmental specialists, Family Service Coordinators, office staff, and other team members to provide exceptional family-centered service.
• Gather and research community resource information and connect families with appropriate programs and services when needed.
• Assist with coordination of transition planning for children and families as applicable.
• Maintain confidentiality and comply with HIPAA, organizational policies, and applicable Early Intervention requirements.
• Complete and maintain required NM FIT Family Service Coordinator training and certification requirements.
• Meet established position benchmarks and program expectations.
• Perform other duties as assigned.

What We Are Looking For (Requirements)

• Education: Bachelor's degree or higher in an appropriate field of study recognized by the NM FIT Program.
• FIT Requirements: Ability to complete and maintain required NM FIT Family Service Coordinator coursework and certification requirements necessary to provide and bill for services.
• Experience: Experience with case management, Early Intervention, child development, family services, healthcare coordination, or children's programs is preferred.
• Language Skills: Bilingual English/Spanish skills are required.
• Organization: Detail-oriented and highly organized with the ability to manage multiple families, appointments, documentation requirements, and program deadlines.
• Critical Thinking: Strong problem-solving and critical-thinking skills with the ability to gather relevant information and coordinate appropriate services for children and families.
• Communication: Strong interpersonal and customer-service skills with the ability to clearly explain programs, processes, and requirements to families.
• Independence: Self-motivated and comfortable independently managing responsibilities while collaborating closely with a multidisciplinary team.
• Adaptability: Ability to work effectively in a changing environment and respond to the individual needs and circumstances of each family.
• Transportation: Reliable transportation, a valid driver's license, and appropriate automobile insurance are required due to community-based travel.

Work Environment & Physical Requirements

This is a community-based position requiring regular travel to meet with children and families in their natural environments, including family homes and other community locations.

The position requires the ability to:

• Travel throughout the assigned service area and safely operate a motor vehicle.
• Work in a variety of home, community, and office environments.
• Lift and carry materials or equipment weighing up to 40 pounds.
• Stand, walk, sit, bend, stoop, crouch, kneel, and reach as necessary when completing job responsibilities.
• Navigate stairs, ramps, uneven surfaces, and other conditions that may be encountered during home and community-based visits.
• Maintain the mobility necessary to safely and effectively interact with infants, toddlers, families, and team members.
