This position supports the Health Homes program, which provides care coordination for members with complex, chronic conditions. It’s a critical role for meeting state contractual requirements and ensuring compliance with TEAMonitor standards.
As a Program Consultant, you’ll lead statewide training for Care Coordination Organizations, provide clinical consultation, and supervise internal care coordinators. You’ll manage audits of Health Homes charts, monitor compliance, and collaborate with behavioral and physical health teams. The role also involves developing policies and procedures, handling tier exception requests, and ensuring accurate documentation and reporting.
To be considered, you must live in Washington State. We’re looking for someone with experience in Medicaid Health Homes or similar programs, strong knowledge of compliance and auditing, and the ability to train large networks. Clinical consultation experience, excellent organizational skills, and familiarity with documentation platforms like HIP are highly valued.
