Certified Aging in Place Specialist (CAPS) Case Manager & Family Disability Navigator
Full-Time | Non-Exempt

About the Role
We are seeking a dedicated and compassionate professional to serve in a dual full-time role, spending approximately 50% of their time as a Certified Aging in Place Specialist (CAPS) Case Manager and 50% as a Family Disability Navigator (FDN). This unique position supports both adults and families of children with disabilities, promoting independence, safety, access to resources, and improved quality of life.
Key Responsibilities
CAPS Case Manager (Approx. 50%)
- Conduct comprehensive in-home assessments to identify accessibility challenges, safety hazards, fall risks, and needs related to ADLs and IADLs.
- Recommend, prioritize, and specify home modifications, including detailed placement requirements (e.g., grab bars).
- Perform follow-up home visits to evaluate modification effectiveness and ensure safety and accessibility standards are met.
- Inspect completed modifications for compliance and issue assessments for any required corrections.
- Accurately document assessments, notes, communications, and follow-up activities in organizational documentation systems.
Family Disability Navigator (Approx. 50%)
- Support children with disabilities (ages 3–22) and their families in navigating the school system, including IEP and 504 processes.
- Conduct needs assessments and develop Individual Living Plans.
- Provide case management and connect families with appropriate community resources and benefits.
- Assist with applications for disability-related services and supports.
- Maintain timely, detailed case notes and documentation.
Qualifications
- Certified Aging in Place Specialist (CAPS) certification preferred, not required.
- Experience working with individuals with disabilities or in case management preferred.
- Strong organizational, communication, and documentation skills.
- Ability to manage a caseload that serves both adults and families.
- Technical proficiency and strong time-management abilities.
- Valid driver’s license required; must be willing to drive a company vehicle.
- Travel required approximately 40–50% of the time for in-home assessments.
- Bilingual Spanish preferred but not required.
