Essential Job Functions:
- Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
- Conducts initial assessment and evaluation to determine eligibility for services.
- Complete Bio-psychosocial assessment.
- Establish individualized treatment plan with each patient and review as clinically indicated.
- Respond, as needed, to patient grievances and complaints.
- Develop aftercare and discharge plans.
- Other related duties as determined by supervisor.
- Must be a licensed in the state of INDIANA Licensed Clinical Alcohol Counselor (LCAC), Licensed Addiction Counselor (LAC), Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW)
- Must have a Masters degree in a qualifying field.
