Apple Valley Congregate Care is seeking a dependable, compassionate, and clinically strong Licensed Vocational Nurse (LVN) to join our skilled nursing team. This position is scheduled for Friday and Saturday AM shifts, with opportunities to pick up additional shifts as staffing needs arise.
Our facility specializes in caring for residents with higher-acuity medical needs, making this an excellent opportunity for an LVN who enjoys providing skilled, hands-on nursing care in a supportive environment.
Responsibilities
- Provide direct nursing care to residents with complex medical conditions.
- Complete thorough nursing assessments and monitor for changes in condition.
- Administer medications and treatments safely and accurately.
- Communicate effectively with physicians, RNs, families, and the interdisciplinary team.
- Maintain accurate, timely documentation in PointClickCare (PCC).
- Respond appropriately during emergencies and changing resident conditions.
- Ensure compliance with all state regulations and facility policies.
Qualifications
- Current California LVN license in good standing.
- Current BLS/CPR certification.
- Skilled Nursing, Subacute, LTAC, or CLHF experience preferred.
- Experience caring for medically complex or higher-acuity residents is highly preferred.
- PointClickCare (PCC) experience is a plus.
- Strong assessment, critical thinking, and communication skills.
- Ability to work independently while collaborating with the healthcare team.

Schedule
- Part-Time
- Friday & Saturday – AM Shift (12-hour shifts)
