Position Summary Information
Minimum Qualifications
Master’s Degree in Social Work
Must pass a criminal background check

Essential Job Functions
○ Provides pediatric clinical mentorship and structured “shadowing” hours for BSW and MSW students.
○ Coordinates student observation/practicum experiences in pediatric neurodevelopmental assessment and early intervention.
○ Conduct psychosocial assessments with families of children referred for developmental evaluations or follow-up.
○ Provide supportive counseling to parents and caregivers coping with new diagnoses, developmental concerns, and care needs.
○ Assist families in accessing early intervention, therapy services, community programs, and benefits (e.g., SSI , Medicaid, IDEA Part C).
○ Serve as a family advocate in multidisciplinary care planning and meetings.
○ Collaborate with pediatricians, therapists, and family navigators to ensure coordinated, family-centered care.
○ Identify and address barriers to care including housing instability, food insecurity, transportation, and mental health needs.
○ Maintain accurate and confidential documentation in accordance with clinic policies and licensing standards.
○ Participate in clinic case reviews and contribute to care planning and follow-up strategies.
○ Track referrals and other data as needed for quality assurance and reporting purposes.
○ Work with Parent 2 Parent Social Worker and Family Navigators to ensure families are supported and connected to appropriate resources.
Preferred Qualifications
○ Experience with high-risk infants, NICU graduates, or children with autism/developmental delays ○ Bilingual or multilingual (especially Spanish or languages commonly spoken in your community) ○ Familiarity with IDEA Part C, IEP / IFSP processes, and family navigation models
○ Trauma-informed care training or experience

M-F, 8:00 am – 5:00 pm
