Summary:
Responsible for managing assigned MDS caseload. Assists the Clinical Coordinator with coordination and supervision of nursing staff throughout the facility. Communicates with resident, family, physician, administration, and other health care providers to meet the total needs of the resident.
Sign-On Bonuses: This role is eligible for $1,000 sign-on bonuses at 3 months, 6 months, and 1 year of full-time employment!
Primary Duties:
- Responsible for thoroughly and accurately completing assigned MDS caseload in an efficient and timely manner
- Assures quality of care through ongoing resident assessment and appropriate intervention, notifying appropriate people regarding resident issues and needs
- Assists in planning of care to attain and/or maintain the highest possible level of health, independence, and mobility of the resident
- Supervises and evaluates care given throughout facility to assure efficient functioning of the facility and improve quality of care
- Communicates with physician and other health care providers to facilitate the overall health care of the resident, managing care for residents on various nursing levels (hospice, skilled care, advanced nursing care, and nursing care)
- Records and reviews progress notes regularly to assure continuity of care and provide up-to-date information for physicians, consultants, nursing staff, and other health care providers
- When the Clinical Coordinator is absent, assists in the supervision of nurse assistants and nurses
- Assists with admissions and discharges as needed
- Attends care plans of residents
- Performs therapeutic procedures in compliance with physician orders and plan of care
- Provides resident and family education where needed to enhance cooperation and undertaking of the care plan
- Participates in continuing education within and outside the facility to improve nursing skills and keep abreast of nursing developments
- Assists nursing staff/CNAs with resident care and needs (transfers, incidents, behaviors, complaints, etc.), spending all excess time providing floor nurse and CNA support
- Manages scheduling shortages, notifying scheduler or initiating contact to nursing staff to fill shortages when scheduler is out of the office
- Coordinates with Nurse Supervisor to ensure processes for new and returning residents are finalized for all admissions during scheduled shift
- Assume the role of floor nurse to manage staffing shortages
- Maintains standards of quality as set by ACR policies and state regulations
- Performs other duties as assigned
Requirements:
- Must be a graduate of an accredited school of nursing with an active Illinois license as a Registered Nurse
- Previous MDS experience required
- Must be able to type at 40 WPM and possess basic computer skills
- This is a full-time, 40 hour/week role. Shifts are either 7:00am-3:30pm or 8:00am-4:30pm five weekdays as well as every four weekend and a holiday rotation. During weeks in which this role works a weekend, they will be scheduled weekdays off in order to remain at five days total per week.
Physical Requirements:
- Considerable physical stamina required for walking and standing often throughout the day with long periods of sitting
- Frequent stooping, stretching, bending, and reaching
- Routinely required to push/pull/lift and/or move up to 40 lbs.
- Some exposure to potentially hazardous equipment, chemicals, and infectious waste
