Position Overview
APluscare is currently hiring a Registered Nurse (RN) to support individuals receiving services in residential and in-home settings throughout East Brunswick and the Central New Jersey region. This role involves delivering clinical oversight, coordinating healthcare services, and supporting staff in providing safe and effective care to individuals with developmental disabilities.
The RN will play a key role in monitoring client health needs, ensuring proper medication management, and collaborating with interdisciplinary teams to maintain high standards of care.

Position Overview

This role involves delivering clinical oversight, coordinating healthcare services, and supporting staff in providing safe and effective care to individuals with developmental disabilities.
The RN will play a key role in monitoring client health needs, ensuring proper medication management, and collaborating with interdisciplinary teams to maintain high standards of care.
Key Responsibilities
- Develop and oversee clinical care plans for individuals receiving in-home and residential services.
- Provide direct nursing support and coordinate healthcare services for clients.
- Conduct new client intake assessments and assist with onboarding of new individuals into services.
- Educate and train staff on required medical procedures, health protocols, and medication administration practices.
- Maintain accurate documentation of clinical services and client health records.
- Oversee medication administration and audit medication administration records (MARs) to ensure compliance.
- Conduct wellness checks and health monitoring as needed.
- Work collaboratively with interdisciplinary teams, including physicians, therapists, and support staff, to ensure coordinated care.
- Maintain strong communication with team members to support effective care planning and service delivery.
- Demonstrate flexibility in responding to the needs of the program and individuals served.
- Participate in Interdisciplinary Team (IDT) meetings to discuss care plans and service updates.
- Experience working with individuals with developmental disabilities is preferred.
- Active Registered Nurse (RN) license in the State of New Jersey.
- Valid New Jersey driver's license.
- Ability to lift, push, or pull up to 50 pounds when necessary.
- Strong communication and collaboration skills.
- Prior experience supporting individuals with developmental disabilities is a plus.
