Job Description:
Join our compassionate team as a Home Care Rehabilitation Therapist, dedicated to making a positive impact on the lives of individuals in the comfort of their homes. As a Home Care Therapist, you will focus on enhancing clients' independence and quality of life through personalized therapy plans.
Key Responsibilities:
1. Conduct in-home assessments to evaluate clients' daily living needs and challenges.
2. Develop and implement customized rehabilitation therapy plans to address clients' specific goals.
3. Provide therapeutic interventions to improve clients' ability to perform daily activities.
4. Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
5. Educate clients and their families on adaptive techniques and equipment for increased independence.
6. Maintain accurate and detailed documentation of assessments, interventions, and progress.
Qualifications:
1. Valid/Active Degree from an accredited program.
2. New York State licensure as an OTR, PT or SLP
3. Strong clinical and communication skills.
4. Ability to adapt therapy plans to diverse client needs.
5. Compassionate and patient-centered approach to care.
6. Valid Driver’s License and/or reliable transportation.
7. Valid Liability Insurance
Experience:
Previous experience in home care or a related field is preferred. New graduates with a passion for home care are welcome to apply.
