RN Case Manager - Acute Care

We are seeking a dedicated RN Case Manager - Acute Care to join our healthcare team in Homer, AK. In this role, you will coordinate comprehensive patient care plans, ensuring seamless communication among multidisciplinary teams to promote optimal health outcomes.

• Assess patient needs upon admission and develop individualized care plans tailored to acute care settings.
• Coordinate hospital services, community resources, and follow-up care to facilitate safe transitions and discharge planning.
• Monitor patient progress, adjust care plans as needed, and advocate for patient needs and preferences throughout their stay.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to deliver coordinated, patient-centered care.
• Provide education to patients and families regarding treatment plans, medication management, and post-discharge instructions.
• Maintain accurate documentation and ensure compliance with healthcare regulations and policies.
