Role: The Primary Care Patient Care LPN works under the primary care operations team to deliver safe, effective, and patient-centered care in collaboration with providers and the clinical team.
Key Responsibilities: Partner with patients and staff to drive quality outcomes, perform duties consistent with organizational standards, and support systematic care delivery.
Qualifications: Graduate of an accredited nursing program; RN or LPN license in the relevant state; preferred 6 months clinical experience, ideally in primary care.
Physical & Certification Requirements: Ability to lift, push, and pull 50 pounds; prolonged standing and walking; certifications include BLS, with additional certifications based on unit needs, such as ACLS or PALS.
Skills & Attributes: Strong clinical skills, ability to work effectively in a team, physical stamina, and adherence to safety standards.
