Occupational Therapist (Sign-On Bonus)

Role Overview

An Occupational Therapist provides personalized, one-on-one therapy to older adults in their homes or senior living communities, aiming to improve mobility, independence, and quality of life. The role involves conducting evaluations, developing care plans, delivering evidence-based treatments, and documenting within electronic medical records. Therapists collaborate with interdisciplinary teams, educate patients and caregivers, and may incorporate telehealth to enhance care access and flexibility.

Key Responsibilities

• Deliver in-home and community-based occupational therapy sessions
• Perform assessments, create individualized treatment plans, and provide evidence-based interventions
• Document patient progress in compliance with standards
• Educate patients, caregivers, and staff on safety and aging in place
• Utilize telehealth when appropriate to extend care
• Work collaboratively within a multidisciplinary team
  
  

Qualifications & Skills

• Valid Occupational Therapy license or eligibility in the practice state
• Degree from an accredited OT program
• Proficiency with computers and EMR systems
• Current CPR certification
  
  

Physical & Other Requirements

• Ability to perform physical tasks such as lifting, repositioning, and assisting patients
• Capability to drive to patient locations and wear PPE as needed
