This full-time position combines in-clinic and remote work, requiring the candidate to work three days in the Jacksonville, FL clinic and two days remotely as a Digital Health Specialist.
Responsibilities Include
- Providing personalized physical therapy care for patients with orthopedic and neurologic conditions.
- Developing and implementing treatment plans, measuring patient outcomes, and documenting evaluations and progress.
- Delivering telehealth services, including evaluations, assessments, and remote therapeutic monitoring (RTM).
- Utilizing digital platforms for virtual care, monitoring, and communication with patients and providers.
- Serving as a liaison between patients, in-clinic providers, and the Digital Health team to optimize remote services.
- Supporting program development, data analysis, and marketing efforts related to digital health initiatives.
- Diploma from a CAPTE-approved Physical Therapy Program and FL state license (or in process).
- Minimum of 2 years outpatient clinical experience; digital health experience preferred.
- Strong commitment to quality patient care and effective communication skills.
- Delivering in-person and telehealth physical therapy services.
- Supporting the growth and development of digital health programs.
- Monitoring patient progress and adapting treatment plans accordingly.
- Collaborating with healthcare providers and the Digital Health team to enhance remote care delivery.
